The Clinical Assessor, in collaboration with CSP staff, provides referrals to VA medical and mental health treatment and to other VA services including VA benefits, community-based social services and other non-VA entitlement programs. If appropriate, he/she provides referrals or consults, or in some cases facilitates direct admission of a Veteran to other VA programs including but not limited to the Community Residential Care Program, Respite Program, Community Health Care Coordination Program, Hospice, Home Based Primary Care, Telehealth and the Community Nursing Home Program. The Clinical Assessor serves as a consultant for navigating the complex processes caregivers, Veterans, and families encounter when interacting with government and community agencies. As a Clinical Assessor within the Caregiver Support Program, the employee stays informed regarding current federal and state legislation directed at assisting caregivers to continue to provide caregiving in an environment that is safe for both themselves and the Veteran. The Clinical Assessor will maintain a knowledge base of current theory and treatment modalities by reading professional journals, attending conferences, clinic seminars and teleconferences, and materials distributed through VA and private sector sources. The Clinical Assessor will collaborate with the VA Patient Aligned Care Team (PACT), mental health, and geriatric primary care clinicians to provide Veteran and staff education on caregiver issues and to obtain input on Veteran's functional needs. Coordination will include chart review, case consultation, in-person, phone or electronic communication, and training of patient aligned care team clinical social work staff. The Clinical Assessor will respond to inquiries from the Centralized Eligibility and Appeals Team and will be responsible for tracking and processing Functional Assessment consults. Develops and fosters relationships with community partners to assist with system navigation, maintain collaboration and coordinate services Maintains basic knowledge of local VA medical center programs, community services and VA Central Office caregiver support initiatives. Maintains knowledge of current federal and state legislation related to caregiver initiatives. The Clinical Assessor performs a wide range of services and may provide technical assistance to state, local, and private agencies, and universities. Technical assistance is based on monitoring activities and program reviews, and the development and analysis of information. In addition, the Clinical Assessor uses sound judgment in the interpretation of trends and patterns; the application of guidelines; and the recommendation of program enhancements. The Clinical Assessor will function in support of caregivers, Veterans and family members throughout the Beckley VA Medical Center and in the community. The Clinical Assessor complies with and supports any ongoing monitoring efforts of the standardized assessment instrument by VA. The Clinical Assessor complies with the requirements of extensive Talent Management System (TMS) training, bi-annual re-training and any ongoing inter-rater reliability and/or peer reviews as determined by policy or national program office guidance. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Experience:
A Bachelor's Degree in Nursing or a related field from an appropriately accredited school is highly desired. A Certification in Ambulatory Care is preferred but is not required. The RN Clinical Assessor must possess the skills needed to effectively work with others to communicate directions and offer assistance. In addition, he/she must demonstrate well-developed organizational skills and require minimal direct supervision. He/she must obtain and maintain Basic Life Support (BLS) certification. Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. The physical requirements of this position include lifting, prolonged standing and sitting, frequent bending, walking (distance), climbing (steps, ladder, etc), reaching (overhead, extensive/repetitive), repetitive motion and stooping. The mental/sensory requirements include recall, reasoning, problem-solving, hearing, speaking clearly, writing legibly, keyboarding, reading and logical thinking.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
